Our player, as well as the parent/ guardian making this request are all members in good standing.

I understand the equipment being borrowed is the responsibility of the parent/ guardian making this request.

The equipment will be taken care of and used for the proper intent on ice only. Equipment will be returned in a timely manor at the end of the season, or before leaving our association for any reason, in reasonably the same condition as it was borrowed.
