Once submitted this form will automatically be sent to the appropriate South Muskoka Minor Hockey representatives to ensure; proper course certification has been obtained; team can be rostered with Hockey Canada; your players names will be submitted to your website; Police Check and declaration submission can be tracked for Team Officials; records are maintained for player movement regarding any fees owing; as well as any volunteer hour reimbursements for approved Team Officials.
