**Time Sensitive** Vaccination Documentation
Hello Bears Families,

In light of the OMHA announcement for proof of a Covid Vaccination, we are now in the position to ask for that proof in order for your player’s registration to remain valid.  We have been asked by OMHA to NOT collect personal documentation but they do require us to see that documentation so we can record it on a spreadsheet and keep your player’s registration valid....

IF and ONLY IF you feel comfortable to VOLUNTEER your vaccine receipt/documentation proving your player has had the Full Dose of the Covid Vaccine, you can email this to our privacy officer, Shannon Zedic (vice president) at [email protected].  Once she has recorded that she has seen it, your documentation will be deleted.  

If you would prefer to send a paper copy, you may drop it off at the Bracebridge arena, in our locked mailbox.  Once it is recorded she has seen it, it will be shredded.  

If neither of these options are available to you, then we will set up limited in person time slots so you can show your documentation to our privacy officer.  We are reminding you that we are not writing down or keeping any personal information written on the vaccination receipt. We are simply putting a checkmark beside your players name to validate their registration.

Although OMHA has given us until October 31st to complete this, the province has also given deadlines to various facilities to start asking for proof of vaccination.  Both Bracebridge and Gravenhurst arenas will need this proof of vaccination from all people ages 12+ entering the building.  Therefore, we need to gather vaccine information from players, coaches, referees and other bench staff by SEPTEMBER 22nd, 2021.  This is a non-negotiable date as players will not be allowed in the arena at that time. 

We recognize that some players may have only received their first dose and this may conflict with potential skates and tryouts.  We do not have an answer for you yet.  Please forward the documentation you have to Shannon Zedic and as soon as we have a timeline answer, we will pass that on.
