**Complaint Process and Feedback Form Instructions**
To help SMMHA streamline and address any issues efficiently, please follow the steps below before completing our feedback form, in accordance with our MOP protocol:
**Step 1: Wait 24 Hours**
- Allow a 24-hour period before proceeding to Step 2.
**Step 2: Contact Your Team Manager**
- Reach out to your team manager with any questions or concerns. If your issue remains unresolved, proceed to Step 3.
**Step 3: Local League contact our Director of U9 & Below or Director of U18 & below, and REP contact the Vice President**
- Discuss your concerns with our Directors or Vice President, who will advise you on the next steps.
**Feedback Form Submission**
- ONLY use the Feedback Form after completing Steps 1 through 3.
- Ensure the form includes your name and contact information for your concern to be addressed.
Once submitted, our D&E Committee will review your completed form, and someone from the committee will be in contact with you shortly.
**PLEASE NOTE:** If all required steps have not been completed, you will be asked to do so before your feedback is reviewed.
